Standard University Agreements
The Office of General Counsel (OGC) and Procurement Services have partnered to streamline the process of negotiating and processing contracts for routine transactions.
The OGC has drafted these Standard University Agreements for use in the following:
- Contracting for the procurement of goods or services.
- Allowing non-university entities to use university facilities.
The Standard University Agreements have been designed to simplify the contracting process by providing secure and fillable forms. No modification of these forms may be made without the prior approval of OGC or Contract Review Services.
INSTRUCTIONS
Submit the completed agreement, termination or amendment through the Procurement Contracts portal for review and processing for signature. Please refer to the Procurement Contract Manual for instructions.

Facility | Athletic Facility Use Agreement-Non-University User | Used by non-university entities wanting to use University Athletic facilities. This agreement requires that licensees provide proof of liability insurance with the University named as an insured party. |

Facility | Facilities Use Agreement | For use of non-classroom and non-laboratory space at the University. Please refer to REG 07.25.12 – Solicitation and REG11.55.02 – Assemblies, Events, and Public Addresses for additional information on this agreement. |
Facility | Lab Use Agreement | For use of laboratory space at the university.The completed agreement must be signed in acknowledgment by the college Dean or Associate Dean for Research and the Vice Chancellor for Research and Innovation prior to being submitted to the Procurement Contracts Portal. |
Facility | IEI Commons Use Agreement | Used by NC State colleges and departments desiring to reserve the common areas of the James B. Hunt, Jr. Library. For assistance in completing this form please contact the events manager of the Institute for Emerging Issues at 919-515-7741. |
General | Filming/Photography Location Agreement | For filming or photographing a university location. This agreement requires that the user carry insurance and name the university as an additional insured. For assistance in completing this form please contact University Communications at 919-515-3871. |
General | Contract Amendment | Used to amend any of the University’s “standard format” agreements by listing the changes (e.g., cost, term, etc.) being made to the agreement. Original agreement must accompany the Amendment. Signatory authority remains with authorized signers of original agreement. |
General | Contract Termination | Used to terminate any of the University’s “standard format” agreements. Signatory authority is granted to the University official who signed the original contract. |
Independent Contractor | Business Associate Addendum | Used primarily by Student Health Services, the Counseling Center and Sports Medicine Department. |

Independent Contractor | Health Care Services Agreement – Counseling Center | For use by the NC State Counseling Center to contract for professional services. Under this agreement the contractor is required to provide proof of medical professional liability insurance at no cost to the University. |
Independent Contractor | Health Care Services Agreement | For use by the NC State Student Health Service to contract for the professional services. Under this agreement the contractor is required to provide proof of medical professional liability insurance at no cost to the University. |
Independent Contractor | Independent Contractor Agreement | For use by the NC State departments and units to contract for a wide range of independent contractor services. NOTE: Current University employees are not eligible to be paid as independent contractors. |
Independent Contractor | Marketing and Communications Services Agreement | For use by NC State departments and units to contract for marketing and communications services. The completed agreement must be signed in acknowledgment by University Communications and Marketing prior to being submitted to the Procurement Contracts Portal. |
Independent Contractor | Instruction Agreement | To contract for non-credit short term projects or instruction assignments. This agreement should not be used to hire instructors to teach for-credit classes. It is a modification of the Independent Contractor Agreement. |
Independent Contractor | Performer Agreement | For use by the NC State departments and units for hiring individual performers or groups to perform on campus. |
Independent Contractor | Theatre Agreement | For use by NC State for hiring artists to perform at Thompson or Stewart theatres. |
